Location: Wilce Student Health Center, Room 064
International & ALP Students will receive Health Screens and TB testing.
Please bring your BuckID with you to the screening along with immunization information and previous TB testing. If you do not have a BuckID, bring your passport or other photo id.
When you arrive at the Wilce Student Health Center, please go to the ground floor, room 064. You will be asked to complete an informational form and sign a HIPAA Privacy notice prior to your screening. You can download these forms from our website (or below) and complete them in advance.

- Complete the top portion of this form for identification & billing purposes.
- Notice of Privacy Practices Acknowledgement (PDF)
- Complete this form to acknowledge you have received information on the SHS privacy practices.
***** Be sure to drink plenty of fluids and to eat your regular meals prior to the Health Screen.
